Abstract: A narrative review was conducted of the historical evolution, biomechanical and neurophysiological foundations, therapeutic objectives, and available evidence on exercise for spinal deviations, with a focus on adolescent idiopathic scoliosis. The presentation integrates international recommendations, systematic reviews, and meta-analyses on specific physiotherapy exercises for scoliosis (PSSE). The main therapeutic approaches—Schroth, SEAS, BSPTS, Lyon, Side Shift, DoboMed, FITS, and FED—are described, and their objectives are analyzed: three-dimensional self-correction, postural control, muscle rebalancing, respiratory retraining, pain reduction, aesthetic improvement, automatization of correction, and integration into activities of daily living. Outcomes are assessed using the Cobb angle, trunk rotation, respiratory function, pain, body image, and quality of life. The combination with a brace, adherence, growth, monitoring, and protocol heterogeneity are also considered.

Conclusions
Current evidence supports the incorporation of postural support exercises (PSSE) into the conservative treatment of mild to moderate adolescent idiopathic scoliosis, either as a standalone intervention or as an adjunct to bracing. These exercises can contribute to stabilizing the deformity, modestly reducing the Cobb angle, improving postural control, function, pain, self-image, and certain components of quality of life. The Schroth method is the most researched, although SEAS and BSPTS also show favorable results. However, mean radiological improvements are usually below the conventional five-degree threshold, and findings regarding trunk rotation are not uniform. The certainty of the evidence remains limited by small sample sizes, heterogeneous protocols, short follow-up periods, risk of bias, and limited information until the end of growth. Multicenter, well-controlled clinical trials with standardized interventions are needed to define the dose, duration, and patient profile most likely to respond.
